About Cornerstone Family Care 

Cornerstone Family Care is a direct-pay Florida based healthcare company providing telemedicine visits to patients who are located only in the state of Florida. Cornerstone Family care is owned and operated by a Florida licensed Autonomous Nurse Practitioner, Amanda Adams, APRN, FNP-C, ENP-C, PMHNP-C. As allowed by Florida law, I do not have a supervising physician and I practice independently within my scope of practice. Cornerstone Family Care can provide comprehensive family care through HIPAA secure telemedicine visits — offering same-day appointments, transparent pricing, and thoughtful treatment plans without the long waits.

What is Telemedicine

Telemedicine is the delivery of healthcare services through HIPAA-secure video and audio visits, allowing you to connect with your provider remotely without coming into the office. It is appropriate for many routine, urgent, and follow-up concerns, including common acute illnesses, medication management, new symptoms or concerns, and chronic condition management.

During a telemedicine visit, you interact with your provider in real time using audio and video technology, similar to an in-person appointment, with the ability to discuss symptoms, review history, receive a diagnosis, and develop a treatment plan. Prescriptions, lab orders, imaging, and referrals can be managed electronically when appropriate.

To participate, you need a smartphone, tablet, or computer with a camera and microphone, along with a reliable internet connection or strong cell signal/connection. You should also have access to a private, quiet space to ensure confidentiality and clear communication. No advanced technical skills are required—most visits are conducted through a secure link that you can open directly from your email or text message without downloading complex software.

We do ask that you are not in public places (salons, grocery stores, sporting events, etc) and that you are not driving or operating other machinery or equipment at the time of your visit as this is a safety risk for you as well as others around you. Please be sure you are parked in a safe and secure location at the time of your visit if you must travel during the time you are scheduled to be seen.

Telemedicine offers a convenient, flexible way to access care while maintaining the same standards of privacy, safety, and quality as traditional in-person visits.

Telemedicine services are available only to patients physically located in the State of Florida at the time of their appointment. Care is provided by an Autonomous Florida-licensed nurse practitioner in accordance with state regulations.

I am a Florida Autonomous Nurse Practitioner, triple board-certified in family, emergency, and psychiatric mental health care, with nearly a decade of experience caring for patients from newborns to older adults. My approach is rooted in the belief that healthcare should be personal, accessible, and built on trust.

My clinical background allows me to address both physical and mental health with a comprehensive, whole-person approach. I care for a wide range of conditions, including chronic disease management such as diabetes, insulin pump therapy, hormone optimization, weight management, and common mental health concerns like anxiety and depression. Each care plan is practical, evidence-based, and tailored to fit your individual needs and lifestyle.

I earned my Master of Science in Nursing from South University and later completed a Post-Master’s Certificate in Psychiatric Mental Health at Walden University. My undergraduate degree in Exercise Science from Florida State University continues to influence my focus on preventive care, metabolic health, and long-term wellness.

In addition to clinical practice, I have participated in pediatric migraine and Alzheimer’s research and have mentored nurse practitioner students throughout my career. I am committed to both patient care and advancing the profession.

Outside of medicine, I enjoy spending time with my family at Disney and training for races, including Spartan competitions. I value resilience, movement, and balance as key components of lifelong health.

Direct Pay Prices

New Patient Weight Loss Visit

$150.00

This visit is for new patients seeking weight loss care. We review your prior efforts (diet, exercise, medications, or surgery), along with your medical history, current medications, and allergies. Labs will be ordered to assess metabolic health and identify any underlying conditions. We also discuss treatment options, including medications, benefits, potential side effects, and how to check insurance coverage of these medications for your follow up visit.

Follow up visits for weight loss $75.00

This visit is for follow-up weight loss care and ongoing medication management. We will review your lab results, assess your progress, and determine the most appropriate medication plan based on your response and goals. We will also refine your lifestyle plan, including nutrition, physical activity, and strategies to track and sustain your progress. These visits are typically monthly.

New Patient Visit Establish Care

$130.00

This visit is for patients who are new to our practice and wish to establish care. We will review your complete health history, including medical conditions, medications, allergies, prior treatments, surgical history, and family history.

Please upload any prior medical records to the patient portal before your visit, if available. If you are unable to do so, we will assist you in obtaining them.

New Patient Visit Sick/acute visit

$60

This visit is for new patients seeking care for a single, acute concern (such as a sick visit). It is intended for individuals who have an established primary care provider elsewhere but need timely, same-day evaluation when their usual provider is unavailable.

Common concerns include cold or flu symptoms, sore throat, sinus infections, cough, fever, urinary symptoms (possible UTI), minor rashes or skin issues, minor injuries, nausea, vomiting, diarrhea, and mild asthma or allergy flare-ups.

Established Patient 3 to 4 Problems Visit

$100.00

This visit is for established patients and is designed to address multiple concerns (typically 3–4 conditions). It may include ordering or reviewing labs and imaging, medication review and management, and discussion of related health concerns. This is a more comprehensive visit due to the complexity of managing multiple conditions.

Established Patient 2 Problems Visit

$70.00

This visit is for established patients and is intended to address up to two medical concerns. It may include ordering or reviewing labs and imaging, medication review and management, and discussion of related issues. This visit may also be used to review previously completed comprehensive lab work.

Established Patient 1 Problem or Sick Visit

$50.00

This visit is for established patients and is intended to address one medical concern or an acute illness. It is not designed for ordering or reviewing labs, which require a more comprehensive (two-problem) visit. Medication refills for a single condition may be provided if appropriate.

Payment Policy Direct Pay

Full payment is required prior to the start of your visit to confirm your appointment. We are primarily a cash-pay practice and do not accept most commercial insurance plans at this time.

Depending on your benefits, you may be able to submit a claim for out-of-network reimbursement. Coverage and reimbursement vary by policy, so we recommend contacting your insurance carrier directly to confirm eligibility and obtain claim submission instructions. Reimbursement is not guaranteed.

How to become a Patient with us

Follow the link below to establish care with us and set up your first appointment. If you have general questions before scheduling and appointment you can also contact us by using our contact us page or calling the office at 904-600-3392 during normal business hours.

Prior to your visit, you will receive intake forms that must be completed in advance. These are similar to the forms required for an in-office visit and are necessary for a safe and thorough evaluation of your health concerns. Forms may include demographic information (name, date of birth, address), HIPAA acknowledgments, telemedicine consent, Florida Autonomous APRN disclosure, health history, payment and insurance information, and consent to treat. You will also be required to upload a valid driver’s license or state-issued ID. For minors, documentation verifying parent or legal guardian status is required.

All forms must be completed before your appointment. If they are not completed in advance, your visit will need to be rescheduled. If you need help filling out these forms please reach out to the office during normal business hours.
