New Patients:

Established Patients:

All appointments are conducted through HIPAA-secure telemedicine. Because we are a virtual practice, you can attend your appointment from the comfort of your home—or anywhere within the State of Florida—as long as you have a reliable internet or cellular connection. We do not maintain a physical office for patient visits.

Before Your Appointment

Please be prepared to:

  • Complete your online intake forms before your visit.

  • Upload a valid photo ID.

  • Upload your insurance card (if using insurance).

  • Upload any relevant medical records, laboratory results, or imaging if available.

  • Ensure you are in a location with strong Wi-fi or cellular signal and stay in that location to ensure there are no interruptions during your visit.

  • Join your telemedicine visit from a private location within the State of Florida.
