Cornerstone Family Care is a Florida-based, independently owned primary care telemedicine practice led by Amanda Adams, MSN, APRN, FNP-C, ENP-C, PMHNP-BC. We provide comprehensive, evidence-based healthcare for newborns through older adults through HIPAA-secure virtual visits, making high-quality primary care and family medicine more accessible for patients throughout Florida.
Whether you need preventive care, treatment for an acute illness, chronic disease management, weight management, or ongoing primary care, you'll receive compassionate, personalized care from the same trusted provider at every visit. We offer same-day appointments when available, extended evening hours to accommodate busy schedules, transparent self-pay pricing, and are in-network with many commercial PPO plans, select HMO plans, Direct Medicare, and Direct Medicaid. Because insurance coverage varies, we encourage patients to verify that Cornerstone Family Care is in-network with their specific health plan before scheduling. We continue to expand our insurance network to serve even more Florida patients.

What is Telemedicine
Telemedicine is the delivery of healthcare services through HIPAA-secure video and audio visits, allowing you to connect with your provider remotely without coming into the office. It is appropriate for many routine, urgent, and follow-up concerns, including common acute illnesses, medication management, new symptoms or concerns, and chronic condition management.
During a telemedicine visit, you interact with your provider in real time using audio and video technology, similar to an in-person appointment, with the ability to discuss symptoms, review history, receive a diagnosis, and develop a treatment plan. Prescriptions, lab orders, imaging, and referrals can be managed electronically when appropriate.
To participate, you need a smartphone, tablet, or computer with a camera and microphone, along with a reliable internet connection or strong cell signal/connection. You should also have access to a private, quiet space to ensure confidentiality and clear communication. Joining your visit is simple. Our appointments are accessed through a secure link sent by email or text message—no complicated software or technical expertise required.
We do ask that you are not in public places (salons, grocery stores, sporting events, etc) and that you are not driving or operating other machinery or equipment at the time of your visit as this is a safety risk for you as well as others around you. Please be sure you are parked in a safe and secure location at the time of your visit if you must travel during the time you are scheduled to be seen.
Telemedicine offers a convenient, flexible way to access care while maintaining the same standards of privacy, safety, and quality as traditional in-person visits.
Telemedicine services are available only to patients physically located in the State of Florida at the time of their appointment. Care is provided by an Autonomous Florida-licensed nurse practitioner in accordance with state regulations.

New Patient Weight Management
$150.00
This visit is designed for new patients seeking medical weight management. During your consultation, we'll review your medical history, current medications, allergies, previous weight loss efforts (including nutrition, exercise, medications, or bariatric surgery), and other factors that may affect your health and weight.
When appropriate, laboratory testing will be ordered to evaluate your metabolic health, identify underlying conditions that may contribute to weight gain, and ensure treatment recommendations are safe and effective.
Together, we'll develop a personalized treatment plan that may include nutrition, physical activity, lifestyle modifications, and prescription medications when appropriate. We'll also discuss the benefits, potential side effects, expected outcomes, and insurance coverage of recommended treatment options.
Follow-Up Weight Management Visit
$75
This visit is designed for ongoing medical weight management and follow-up care. We will review your progress, discuss any changes in your health, evaluate your response to treatment, and review laboratory results or other testing, when applicable. Together, we'll determine the most appropriate treatment plan based on your progress, goals, and any medication benefits or side effects you may be experiencing.
We'll also continue to refine your personalized weight management plan by addressing nutrition, physical activity, healthy lifestyle habits, and practical strategies to help you achieve and maintain long-term success. Follow-up visits are typically scheduled monthly but may be adjusted based on your individual treatment plan and progress.
New Patient Visit Establish Care

This visit is designed for patients who are new to Cornerstone Family Care and would like to establish ongoing primary care. During your appointment, we will review your complete medical history, including current and past medical conditions, medications, allergies, surgical history, prior treatments, family history, and any health concerns you would like to discuss. This comprehensive review allows us to better understand your health needs and develop a personalized plan of care.
If available, please upload any previous medical records, recent laboratory results, imaging reports, or specialist notes to the patient portal before your appointment. If you are unable to obtain these records, we will be happy to assist you in requesting them when appropriate.
New Patient Visit Sick/acute visit
$60
This visit is designed for new patients seeking evaluation and treatment for one acute medical concern. It is ideal for individuals who have an established primary care provider but need prompt, same-day care when their regular provider is unavailable or an appointment cannot be obtained in a timely manner.
Common concerns include cold and flu symptoms, sore throat, sinus infections, cough, fever, urinary symptoms (including possible urinary tract infections), nausea, vomiting, diarrhea, mild asthma or allergy flare-ups, minor skin conditions or rashes, and other non-emergency illnesses or injuries that can be safely evaluated through telemedicine.
Established Patient 3 to 4 Problems Visit

This visit is designed for established patients with multiple health concerns, typically 3–4 conditions. We will review your symptoms and overall health, manage medications, order or review laboratory testing and imaging when appropriate, and address any related medical concerns. Because multiple conditions are being evaluated and managed, this visit allows additional time for a comprehensive assessment and personalized treatment plan.
Established Patient 2 Problems Visit
$70.00
This visit is designed for established patients who need to address up to two medical concerns during a single appointment. We will evaluate your symptoms, review and manage medications, order or review laboratory testing and imaging when appropriate, and discuss any related health concerns. This visit may also be used to review previously completed comprehensive laboratory results and adjust your treatment plan as needed.
Established Patient 1 Problem or Sick Visit
$50.00
This visit is designed for established patients with one medical concern or an acute illness. During your appointment, we will evaluate your symptoms, provide treatment recommendations, and manage medications related to that condition when appropriate. Because this visit focuses on a single concern, it is not intended for ordering or reviewing laboratory tests, which require a more comprehensive appointment.
Full payment is required prior to the start of your visit to confirm your appointment. We are primarily a cash-pay practice and do not accept most commercial insurance plans at this time. Depending on your benefits, you may be able to submit a claim for out-of-network reimbursement. Coverage and reimbursement vary by policy, so we recommend contacting your insurance carrier directly to confirm eligibility and obtain claim submission instructions. Reimbursement is not guaranteed.

How to become a Patient with us

Prior to your visit, you will receive intake forms that must be completed in advance. These are similar to the forms required for an in-office visit and are necessary for a safe and thorough evaluation of your health concerns. Forms may include demographic information (name, date of birth, address), HIPAA acknowledgments, telemedicine consent, Florida Autonomous APRN disclosure, health history, payment and insurance information, and consent to treat. You will also be required to upload a valid driver’s license or state-issued ID. For minors, documentation verifying parent or legal guardian status is required.
All forms must be completed before your appointment. If they are not completed in advance, your visit will need to be rescheduled. If you need help filling out these forms please reach out to the office during normal business hours.
